Is lactose-free milk low FODMAP? Not always

Yes, lactose-free milk is low FODMAP. But "lactose-free" and "low FODMAP" don't mean the same thing. A lactose-free product can still contain other FODMAPs. Here's how to tell the difference.

Short answer: yes, lactose-free milk is low FODMAP.

But that doesn't mean every product labelled "lactose-free" is automatically low FODMAP.

Lactose is just one type of FODMAP. Remove or break down the lactose, and other FODMAPs can still be present depending on the ingredients.

Understanding that difference makes navigating the dairy aisle a lot easier.

Lactose is just one type of FODMAP

FODMAP stands for Fermentable Oligosaccharides, Disaccharides, Monosaccharides And Polyols.

These are different types of fermentable carbohydrates that can trigger digestive symptoms in some people with IBS.

Lactose belongs to the disaccharide group. Other FODMAPs include fructans, GOS, excess fructose and polyols.

That's why lactose-free and low FODMAP are not interchangeable terms.

A product can contain no lactose and still contain other FODMAPs from ingredients such as inulin, chicory root, certain fruits, wheat, onion or garlic.

What lactase actually does

Here's where two very similar words can cause a lot of confusion:

Lactose = a type of sugar naturally found in milk.

Lactase = the enzyme that breaks lactose down.

To make lactose-free milk, lactase is added to break lactose down into the simple sugars glucose and galactose, which can then be absorbed in the small intestine.

So seeing lactase on an ingredient list is not a FODMAP red flag. It's actually how many lactose-free dairy products are made.

And yes: lactose-free cow's milk is considered low FODMAP.

Why a lactose-free product can still contain FODMAPs

This is where looking beyond the front of the package matters.

"Lactose-free" only tells you about lactose. It doesn't tell you what else has been added to the product.

For example, a lactose-free yogurt, dessert, sauce or flavoured milk could also contain:

  • Inulin or chicory root, which contain fructans
  • Apple or pear ingredients, which may contribute fructose and/or polyols
  • Honey, which can be high in excess fructose depending on the serving
  • Wheat-based ingredients, which can contribute fructans
  • Garlic or onion, common sources of fructans

Whether the final product is low FODMAP depends on the ingredients, their quantities and the serving size.

So the useful rule isn't:

lactose-free = low FODMAP

It's:

lactose-free = the lactose part is taken care of.

You still need to look at the rest of the product.

Some dairy foods are naturally low in lactose

Following a low FODMAP diet doesn't necessarily mean avoiding dairy altogether.

Some dairy foods naturally contain very little lactose.

Hard and aged cheeses such as cheddar and parmesan, for example, are naturally low in lactose. Other cheeses can also have low FODMAP serving sizes.

Butter is also naturally very low in lactose.

For dairy products that contain more lactose, such as regular milk or some fresh dairy products, serving size becomes particularly important.

What about plant-based milks?

Plant-based doesn't automatically mean low FODMAP either.

Several plant-based milks have low FODMAP serving sizes, but the type of milk, ingredients and portion size all matter.

Soy milk is a good example.

Soy milk made from soy protein can be low FODMAP, while soy milk made from whole soybeans is higher in GOS.

Oat milk can also be portion-dependent, and recipes vary between brands.

Added ingredients matter too. Inulin and chicory root fibre, for example, can increase the FODMAP content of an otherwise suitable product.

So whether you're choosing dairy or plant-based milk, the same principle applies: check the product, not just the category.

Lactose intolerance and IBS are not the same thing

Lactose malabsorption happens when the small intestine doesn't produce enough lactase to fully digest lactose.

IBS is different.

Someone with IBS may tolerate lactose perfectly well but react to other FODMAPs. Someone else may be sensitive to lactose as well as other FODMAP groups.

This is also why simply cutting out dairy doesn't necessarily identify what is causing digestive symptoms.

The low FODMAP diet is designed as a temporary process of restriction followed by structured reintroduction to identify individual tolerances, ideally with guidance from a FODMAP-trained dietitian.
